" Until December 31, 2019, contract doctor practices with more than 3 contract doctors (cumulative full-time equivalent) also have the aforementioned lump-sum entitlement to one-time billable complexity surcharge, which doubles for contract medical practices with more than 6 doctors (cumulative full-time equivalent). “
