56. Amendment of the agreement on forms for medical care by contracted doctors from April 1, 1995 (Annex 2 BMV-Ã „)
Article 1
Changes to the form agreement
1. Numbers 2.10C and 2.10C.1 to 2.10C.3 are introduced as follows:
a 2.10C Sample 10C: Order for SARS-CoV-2 testing (as of: 06.2020) 1
2.10C.1 As an order for SARS-CoV-2 tests with report The attached sample 10C is to be used for the “increased risk” from the Corona warning app or for diagnostic clarification.
2.10C.2 The sample 10C consists of two sections arranged one below the other, each in DIN A5 landscape format, which can be separated from one another by means of perforation.
2.10C.3 The pattern 10C does not have colored surface pressure. No. 1.1.5 applies accordingly. The form is in A4 format. œ

Changes to the explanations on the form
1. The form explanations for sample 10C are newly inserted as follows:
a Sample 10C: Order for SARS-CoV-2 testing
Only sample 10C is to be used for commissioning SARS-CoV-2 testing using nucleic acid detection. This applies after message of "increased risk" by the Corona warning app or for diagnostic clarification. Sample 10 must not be used for the orders for SARS-CoV-2 tests using nucleic acid detection.
Sample 10C must not be used as copy. Each sample 10C contains an individual GUID in the QR code, which can be transmitted from the laboratory to the server systems of the Corona warning app together with the test result. Provided that the insured has given their consent, they can view their test results by specifying the individual GUID in the QR code.
This referral slip for laboratory tests is divided into two parts. The upper part of the form is used to commission the laboratory and must be completed by the referring contract doctor. The lower part contains the data protection information and the individual GUID QR code for the insured.
