Changes to the agreement (Annex 2a BMV-Ä)
1. In the overview in chapter 1.4, the entries for patterns 60 / E and 61 / E are changed as follows:
2. The numbers 2.60.1 to 2.60.7 are deleted.
3. The numbers 2.60, 2.61 and 2.61.1 to 2.61.13 are changed as follows, the numbers 2.61.14 to 2.61.16 are newly inserted as follows:
" 2.60 Pattern 60 / E: unoccupied 1
2.61 Sample 61 / E: Advice on medical rehabilitation / prescription of medical rehabilitation (Status: 4.2016)
2.61.1 For advice on medical rehabilitation, samples 61Aa / E and 61Ab / E must be used in accordance with the form shown in Sections 2.61.9 and 2.61.10. If the doctor just wants advice or an examination by the responsible rehabilitation provider, only part A must be printed out.
2.61.2 The sample consists of two forms:
Sample 61Aa / E: Copy for the contracted doctor
Sample 61Ab / E: Copy to remain with the health insurance company
< p> Both forms are first sent by the contract doctor to the health insurance company. This sends the original back to the contracted doctor.2.61.3 Samples 61Aa / E and 61Ab / E must be drawn up on A4 format security paper.
2.61.4 The pattern 61Ab / E is provided with barcode. The specifications can be found in the currently valid version of the "Technical Manual for Blank Form Printing".
2.61.5 For the prescription of medical rehabilitation, samples 61Ba / E to 61Db / E are to be used in accordance with the form shown in Sections 2.61.11 and 2.61.16. If the doctor does not want advice or an examination from the responsible rehabilitation provider, only parts B to D are to be printed out.

Sample 61 Part Da / E: Original for the health insurance company
Sample 61 Part Db / E: Copy to remain with the contracted doctor
2.61.7 Samples 61 Part Ba / E to 61 Part Db / E are on security paper in DIN format A4 upright.
2.61.8 The samples 61Ab / E and 61Da / E are provided with barcode. The specifications can be found in the currently valid version of the "Technical Manual for Blank Form Printing".
