Article 1 Changes to the explanations of the form
1. Number 6 of the form explanations for sample 5 changes as follows:
“The patient's gender must be indicated by marking the relevant field with an X. . For patients of undefined gender, both fields must be marked with an X. “
2. Number 4 of the form explanations for sample 6 changes as follows:
“The gender of the patient must be indicated by marking the relevant field with an X. For patients of undetermined gender, both fields must be marked with an X. “
3. The following sentence is added to number 6 of the form explanations for sample 10:
"For patients of undetermined gender, both fields must be marked with an X."
4. Number 5 of the form explanations for the sample 10A changes as follows:
"The gender of the patient must be indicated by marking the relevant field with an X. For patients of undetermined gender, both fields are to be marked with an X. “
5. Number 3 of the form explanations for sample 19 is newly inserted as follows:
“The patient's gender must be indicated by marking the relevant field with an X. For patients of undetermined gender, both fields must be marked with an X. “
6. The following sentence is added to number 1 of the form explanations for sample 30:
"The patient's gender must be indicated by marking the relevant field with an X. For patients of undetermined gender, both fields must be marked with an X. "
Article 2 Coming into force
The changes will come into effect on 01/01/2016.
