The delegates were relieved of the reform of the training has finally been decided, although not all requirements have been met. The next challenge is the creation of sample training order. Climate protection was also one of the topics in Berlin.

We have been warned about the consequences of climate change for 50 years - and we only have one world, ”explained Dr. rer. nat. Dietrich Munz, President of the Federal Chamber of Psychotherapists (BPtK) at the beginning of the 35th German Psychotherapists' Day (DPT) on November 15 in Berlin. There is no shortage of climate protection concepts; political implementation alone is difficult. In addition, the ecological question should not be solved in competition with the economic question, demanded Munz. For example, rents would still have to remain affordable despite energy renovation of apartments. “As the Federal Chamber of Psychotherapists, we cannot position ourselves with the Fridays for Future movement. Every psychotherapist must clarify his position on this for himself. But the direct and indirect consequences of climate change for people are already part of our tasks as chamber, ”explained Munz. Therefore the board decided to give the movement “Psychologists / psychotherapists for Future” (Psy4F) the opportunity to present their positions to the delegates.

Failure to act as problem
Mareike Schulze, psychotherapist from Rhineland-Palatinate, took over this task about six months ago, together with the psychotherapist Lea Dohm launched the Psy4F movement (see interview in PP , issue 9/2019). “The climate crisis has an impact not only on physical health, but also on mental health: due to the threat, fears, depression, stress, post-traumatic stress disorders and pretraumatic stress are increasing," explained Schulze. The social consequences of climate change are particularly noticeable for poor and old people as well as for children. Nevertheless, many people do nothing to counteract the climate catastrophe. "It is also psychological problem that so many people do not act," said Schulze.

"We demonstrate, mobilize and support stressed colleagues," reported the activist. “And we think it is professional ethical obligation to deal with the climate crisis. As profession, we should think about what we can do. ”In resolution on climate protection and sustainable action, the delegates of the 35th DPT called on (box ).
In addition, passed motion to the board to develop “long-term sustainability strategy” for the BPtK. The principle of sustainability should be reflected in the concrete actions of the BPtK office, in board and committee work as well as in the compensation and travel expense regulations. In addition, the Chamber's participation in the social debate and cooperation with other actors should be examined. Key points will be presented at the next DPT.
Anti-Semitism and Racism
The Chamber President addressed another socio-political problem in his Management Board report: increasing racism and anti-Semitism. "Jewish people are attacked in Germany, Muslims are discredited - that is the beginning of the end of civil society," said Munz. Psychotherapy is inconceivable in society that cannot guarantee the rights of the individual - and that is also why psychotherapists have to deal with the dangerous development. Discrimination against mentally ill refugees must be resolutely countered. In request to the federal government (Drucksache 19/14807) on November 6, the AfD asked, among other things, about mentally ill offenders who came to Germany as refugees and the number of their crimes. "Words can work like small arsenic cans", stressed Munz, and: "We shouldn't underestimate the seriousness of the situation: first we say, and then we do." sensitive point from the digital supply law (DVG), which the Bundestag passed on November 7th, under the microscope. The law enables doctors and psychotherapists to prescribe digital health applications for their patients at the expense of statutory health insurance (see also page 539). The Federal Institute for Drugs and Medical Devices (BfArM) is to set up directory with digital health applications that have medical benefit. "We demand that their effectiveness is proven by scientific studies," said Munz. With the support of Psychologists / Psychotherapists for Future, every individual can do something for climate protection: According to Schulze, 4,200 colleagues have already signed the movement's appeal. The apps are not substitute for indicated treatment. The BPtK also opposes efforts by health insurers to recommend certain apps to their policyholders. For the cash registers, cost savings are in the foreground. But good apps can also be harmful if they are not indicated for the patient. The treatment sovereignty must therefore lie with the psychotherapist. "We have to try to prevent second digital supply offer from the health insurers," demanded the BPtK President. In the debate on this topic, Rudi Bittner, delegate from Bavaria, said: "I will not be able to take any responsibility if untested apps are prescribed." phil. Rainer Richter, Hamburg, pointed out that there has not yet been study on the possible side effects of health apps. "For this money could be obtained from the innovation fund. We should get involved here ”, he appealed to the psychotherapists.

Approval regulations are available
The next big topic that BPtK President Munz dealt with in the report of the board was the reform of training. On November 8, the Federal Council finally passed the Psychotherapist Training Reform Act, which will introduce direct psychotherapy studies with license to practice medicine and subsequent further training from autumn 2020. "Five years ago at the 25th DPT, after intensive discussions, we made fundamental decision in favor of direct training," recalled Munz. “The reform wrested lot from us, we argued lot and came to an agreement again and again. But the efforts were also community-building for us, because externally we were able to appear united. We have achieved lot and we can be proud of that. "Barbara Lubisch, delegate from North Rhine-Westphalia, added:" Our profession is now much stronger. We are much more on par with the doctors. In the National Association of Statutory Health Insurance Physicians (KBV) and in the national associations, we have received lot of recognition for this, ”reported the first deputy chairwoman of the KBV representative assembly.
Despite all the criticism of individual details, the reform is good for the profession . "We get more creative freedom for our profession and can set the course ourselves," emphasized Munz. Rehabilitation, prevention, expert functions, organizational and management tasks will in future also be part of the skills of psychotherapist.Effective health apps could lead to successful treatment if they were used alongside therapy. Rehabilitation, prevention, expert functions, organizational and management tasks will in future also be part of the skills of psychotherapist.Munz described the draft bill for license to practice medicine, which the Federal Ministry of Health presented on October 17, as "basically positive". Although improvements are still necessary in individual points, "it reflects our competence profile and, with the course, leads to license to practice medicine that does justice to the professional title of psychotherapist." Among other things, the license to practice medicine defines “process-wide” qualification. In resolution, the delegates demanded that it should contain “clear minimum requirements to safeguard the variety of procedures and methods enshrined in the law” (box ).
Financing of further training p >
The biggest shortcoming of the Psychotherapist Training Reform Act, according to Munz, is the lack of security for the financing of the two-year outpatient training beyond the remuneration of the treatment services financed by the health insurance fund. Although the law provides for collective wage during inpatient training in psychiatric and psychosomatic clinics, for the outpatient phase "the federal and state governments did not agree on how comprehensive financing could be ensured". Politicians also find it so difficult to finance the outpatient sector because the doctors - apart from the general practitioners - also have no funding for their outpatient training. "If we get funding, the doctors want that too," said Munz. "Our political work is not over at this point." In support of this, the delegates decided on an application, which called on the BPtK board of directors to get the BMG and the legislature to clarify the unresolved financing issue as soon as possible div>
PiA see themselves as Loser
In addition, the BPtK wants to work to improve the remuneration of psychotherapists in training (PiA) who will only finish their training in the coming years. According to the Reform Act, they should be remunerated at least EUR 1,000 per month during the year of practice in psychiatric clinics. The representative of the federal conference PiA, Mechthild Leidl, wanted support from the delegates: "We are the losers of the reform, because 1,000 euros gross is not even minimum wage level, but only fixation of the current situation." connected with the law. PiA should be grouped as university graduates according to collective bargaining law, said Leidl.Munz described the draft bill for license to practice medicine, which the Federal Ministry of Health presented on October 17, as "basically positive". "We'll be around for the next 15 years," she clarified.
The next challenge for psychotherapists is now to develop model further training regulation (MWBO). "Let us develop MWBO that secures the future of our profession, sets standards and perspectives for management functions, regulates new fields of activity and new powers," the BPtK President appealed to the delegates. "We expect the first further training candidates after lateral entry into the master's program from autumn 2022," said BPtK Vice President Nikolaus Melcop. That is why the BPtK must now work at full speed to develop MWBO in structured dialogue with the state chambers of psychotherapists. Michaela Willhauck-Fojkar from the Board of Directors of BPtK presented the basic structures of future training (box) .
In the discussion that followed, Josef Bailer, delegate from Baden-Württemberg, pointed out: “ We also have to seek advice from our medical colleagues who have the experience on what is not going well in inpatient care, and not just from professional politicians. ”Karl-Wilhelm Höffler added that it is also important to ensure that there are sufficient further training positions , Hessen. In general, Georg Schäfer, North Rhine-Westphalia, sees an upgrading of the profession. "But we have only achieved part of what we set out to do," he criticized. The question is how "the grievance in the psychological department" can be overcome.
Overpowering behavioral therapy
The chairman of the German Society for Psychoanalysis, Psychotherapy, Psychosomatics and Depth Psychology (DGPT ) the fact that 59 of the 60 chairs for clinical psychology and psychotherapy in Germany are occupied by behavioral therapists and only one psychoanalytic chair is available. The license to practice medicine must therefore stipulate that the field of psychotherapy is represented “in its entirety” in the course of study and taught by university teachers and lecturers who have the appropriate specialist knowledge. "This has not yet been ensured, so we need revision of the license to practice medicine," explained Schäfer.
In addition to the actual reform of training, the Psychotherapist Training Reform Act also provides for number of changes in the Social Code (SGB) V, which " equates to reform of the supply “, emphasized BPtK President Munz. In particular, the mandate to the Federal Joint Committee (G-BA), anchored in the law, to develop guideline for cross-professional, coordinated and structured care for seriously mentally ill people by the end of December 2020 is very promising. An exemption from school fees is just as necessary. This change was made in consultation with the profession. But not the amendment that was introduced into the law at very short notice to add fee of 15 percent to the first ten hours of short-term therapy. "We are very ambivalent about this idea," said Munz. A higher remuneration for the first more complex hours is generally good. But the underlying idea, namely to make shorter therapies more attractive, will not work. "Psychotherapists treat their patients for as long as it is necessary - we cannot be corrupted."
Expert proceedings should go away
The Chamber President was also disconcerted with regard to another - brought in at short notice - Amendment to the Psychotherapist Training Reform Act: The legislature has commissioned the G-BA to develop minimum standards for quality assurance by the end of 2022 in order to subsequently abolish the application and expert assessment procedure. As result, the prior performance audit of the guideline psychotherapy will be omitted, so that all services will be subject to the usual performance audit for doctors and psychotherapists (§ 106 a, SGB V). “This represents substantial intervention in psychotherapeutic care that should not have been carried out without prior consultation with the profession,” explained Munz. “We will not accept senseless documentation requirements. Our interests and our special features are in the foreground. ”Incidentally, quality assurance is chamber's responsibility. By the next German Psychotherapist Day, the Federal Chamber of Psychotherapists, in coordination with the regional chambers, wants to present recommendations for standard documentation. The 36th DPT will take place on May 15th and 16th, 2020 in Leipzig. Petra Bühring
Diotima Prize 2020 for the fight against pain

In the run-up to the 35th German Psychotherapist Conference, Prof. Dr. rer. nat. Birgit Kröner-Herwig and Dr. rer. nat. Paul Nilges received the Diotima Prize. This year the German psychotherapists are honoring colleague who is committed to caring for people with chronic pain and who have made significant contribution to establishing qualified pain psychotherapy. “17 percent of all Germans suffer from chronic pain - that's more than 12 million people. Their history of suffering lasts an average of seven years, more than 20 percent suffer from chronic pain for over 20 years, ”reported Dr. rer. nat."Many of us have been involved in this area for long time, we have come long way through the G-BA contract - we have to use that," explained Munz. Birgit Kröner-Herwig held the Chair of Clinical Psychology and Psychotherapy in Göttingen until her retirement in 2016. It is awarded to pioneer in psychological pain research in Germany. Already in the 80s of the last century, when the topic of pain was still not very prominent in psychology, she dealt with the psychophysiological connections of chronic pain and the effect of biofeedback on headaches. Kröner-Herwig's research interests also focused on the development and research of effective treatment approaches. In 1990 she became co-editor of the first edition of the textbook "Psychological Pain Therapy", which is now considered standard work under the title "Pain Psychotherapy".

Paul Nilges was the head psychotherapist at the DRK pain center in Mainz until his retirement. In addition to his clinical and scientific work, he has worked with passion and commitment to establish qualified pain psychotherapy in Germany. He recognized early on that the role of psychotherapists in the interdisciplinary care of pain patients with the appropriate qualifications is gaining importance and promoting the integration of psychotherapists in what was originally purely medical field. For the development of pain psychotherapy, his long-standing participation in the German Pain Society, which until then had been dominated by doctors, was very important. With lot of perseverance and persuasiveness, Nilges also fought for the Rhineland-Palatinate State Chamber of Psychotherapists to introduce further training in "Pain Psychotherapy" in 2006 ....
Basic structures of further training
- The five-year advanced training course (WB) is divided into the areas of “Psychotherapy for Children and Adolescents” and “Psychotherapy for Adults” and is linked to the acquisition of professional competence in one or more scientifically recognized psychotherapy procedures. The job title is, for example, “specialist psychotherapist for adults (systemic psychotherapy)”.
- The full-time job is to complete at least two years in outpatient and inpatient care and optionally in institutional care.
