27. Amendment of the agreement on forms for statutory health care

from April 1, 1995

1. The following changes will be made to the form explanations:

1.1. Editorial changes

1.1.1. Model 61: Ordinance on medical rehabilitation In paragraph 4, the quotation mark in front of the closing brackets is deleted.

1.2. Changes in content

1.2.1. Sample 16: Drug Ordinance Sheet

Number 11 is reworded as follows:

"When prescribing formulas, only the front of the form may be used. One regulation sheet must be used for each recipe. Recipes for parenteral use can be prescribed for up to week as required, provided that the individual preparations to be used are identical in terms of type and quantity (e.g. infusion bags). Patient-specific partial quantities (especially weekly blisters) taken from finished medicinal products can be prescribed for up to four weeks as part of long-term medication. "

1.2.2. Model 63: Ordinance on Specialized Outpatient Palliative Care (SAPV)

In number 5, the following is inserted in the first sentence before the closing brackets: "- it is only the back of model 63a (copy for the health insurance company) to be filled in ".

3. This agreement comes into force on October 1st, 2010.
