26. Amendment to the agreement on forms for statutory health care from April 1, 1995

The Kassenrztliche Bundesvereinigung, Berlin, on the one hand -Spitzenverband (Spitzenverband Bund), Kd.R., Berlin, on the other hand, agree the following 26th amendment to the agreement on forms for contractual medical care dated April 1, 1995 1. The following changes are made to the explanations of the forms: 1.1. Editorial changes 1.1.1. Sample 10A: Request form for laboratory tests in laboratory communities In the first sentence, the word referring is replaced by requesting. 2. This agreement comes into effect on April 1, 2010.
