

Die Kassenärztliche Bundesvereinigung, Kassenärztliche Bundesvereinigung, Kassenärztliche - on the one hand - and the GKV-Spitzenverband, Kdö.R., Berlin - on the other hand - agree on the following
from October 1st, 2002
The following changes are made in Chapter 1 of the agreement:
I. The number 1.1.9 changes as follows:
For the production of the security paper, ink-resistant white paper of quality suitable for machine document reading is required, i.e. H. to be used in paper thickness of at least 90 g / m2 in accordance with DIN 6723.
II. The number 1.1.12 changes as follows:
The “Technical Manual for Blank Form Printing” is created and constantly updated by the National Association of Statutory Health Insurance Physicians in agreement with the National Association of Statutory Health Insurance Funds. At the same time as the software houses, the National Association of Statutory Health Insurance Funds is informed of the changes made.
III. Number 1.2.4 changes as follows:
The contract doctor can use samples 11 / E, 17 / E, 25 / E and 55 / E as well as the backs of samples 50 / E bis 53 / E (50.2 / E to 53.2 / E) as blank forms. These can be attached to forms 11, 17, 25 and 50 to 55, which are sent by the health insurance companies or the insured person to the contract doctor and, after he has entered the required information, should be returned in writing to the health insurance company.

2.3 Sample 3 / E: Certificate of the presumed day of childbirth (Status: 1.2014)
2.3.1 For the certificate of the presumed day of childbirth, the specimen according to the in Sections 2.3.7 and 2.3.8 should be used.
2.3.2 The certificate of the presumed day of delivery consists of the following forms:
Sample 3a / E: Copy for the health insurance company
Sample 3b / E: Copy for the insured
