1 The following changes will be made in the agreement:
1.1 Changes to the content of Chapter 1:

1.2 Changes in content in chapter 2:

2.5 Sample 5 / E: Billing slip for outpatient treatment, treatment by attending physicians , Clarification of somatic causes before starting psychotherapy, recognized psychotherapy (status: 4.2011)
1.2.2 Numbers 2.5.1 to 2.5.4 are inserted as follows:
2.5.1 The pattern is 5 / E in the form shown in Chapter 2.5.4 only to be used when using the substitute procedure in order to document the patient's signature with regard to the insured person data specified in the substitute procedure. When printing blank forms on sample 5 / E, only the data in the personal details field and in the field for the contract doctor's stamp may be printed out. The billing-relevant fields must remain empty, as the form is only intended to be left in the doctor's office. Billing must continue to be carried out electronically.
2.5.2 The sample 5 / E must be created on security paper in DIN A5 format.
2.5.3 The sample 5 / E does not contain any Barcode.
